Incident Reporting
Prompt reporting of injuries and accidents helps Sisters School District maintain safe schools, respond appropriately, and identify opportunities to improve safety for everyone. Whether an incident involves a student, volunteer, visitor, contractor, or employee, timely and accurate reporting allows the district to provide appropriate follow-up, document the event, and help prevent similar incidents in the future.
If an injury or accident occurs on school property or during a district-sponsored activity, it should be reported to the appropriate school administrator, supervisor, or event coordinator as soon as possible. When necessary, emergency medical services will be contacted immediately. District staff will assist with the reporting process and ensure appropriate follow-up occurs.
The District Injury / Accident Report is used to document injuries or accidents involving students, employees, volunteers, visitors, contractors, or other individuals while on district property or participating in school-sponsored activities. The report records important details about the incident, including what occurred, where and when it happened, witnesses, and any actions taken following the event.
Prompt reporting helps ensure appropriate medical care when needed, supports accurate documentation, assists with insurance and legal requirements, and allows the district to identify trends and implement improvements that enhance safety for everyone.
